江戸時代の小石川養生所を舞台に、青年医師の保本登と、小石川養生所の医長である「赤ひげ」こと新出去定（にいで・きょじょう）、患者との魂のふれあいが描かれる。
黒澤明監督が『赤ひげ』と題して、三船敏郎主演で映画化した（1965年）ほか、たびたび映像化・舞台化されている。
